SETTING UP AN APPOINTMENT
Most children who are seen in the Civitan-Sparks Neuropsychology Clinic have been referred by a medical doctor or health care professional, due to concerns with neurological processing and functioning. The child’s physician typically contacts our office with pertinent information to set up an appointment for neuropsychological evaluation. Our clinic does accept Medicaid patients. Medicaid patients may instruct their primary medical physician (PMP) to fax the Medicaid Patient First Referral to Civitan-Sparks Clinics, care of Joyce Brown. The Medicaid referral should include the names of the patient, primary care physician, and parent/guardian, as well as the home address and phone number of the patient. The purpose of the referral (neuropsychological evaluation) should also be included on the fax. PROVIDING IMPORTANT INFORMATION
Once a child or adolescent has been placed on our schedule, there are certain steps we must take prior to the evaluation date. For the evaluation to be as thorough and helpful as possible, the clinic will need additional information from the child’s family, as well as other facilities where the child has been seen. The parent will receive forms for the family and the patient’s teachers to fill out, prior to the evaluation. Although these forms do take significant time and effort, we ask parents and teachers to be as thoughtful and thorough as possible when filling out our forms. They are a very important part of our evaluation. It is important that we also receive medical records and any prior psychological, educational, developmental, or intellectual testing which may have been performed in the past. Caregivers are encouraged to provide us with any additional information or records that may be necessary for a comprehensive evaluation.

PREPARING FOR THE TESTING DATE
There are certain guidelines that the parent and child should follow in order to make testing as valid as possible. These guideline include:

  • Try to arrange for your child to have a good night’s rest prior to testing. Some families who travel long distances find it useful to arrange to spend the night in the Birmingham area. Click here for local accommodations information.
  • Your child should have a good breakfast and take any regularly prescribed medication (unless directed to do otherwise) on the day of testing.
  • Your child should have had a vision and hearing screening within the previous 12 months. If you do not have current screenings, please arrange this through your school or physician’s office. You may also schedule these services through UAB. Hearing evaluations at Sparks may be scheduled through Darlyene Graham (205-934-5457) and vision evaluations may be scheduled through the UAB School of Optometry (205-934-3058). If your child has any visual or hearing impairment, please let our staff know prior to the day of testing. Make sure to bring any corrective devices, such as glasses or hearing aids, to the evaluation.
  • Testing can take anywhere from 2-8 hours, so be prepared to stay all day. We generally break for an hour lunch at 12:00. The parent should also be prepared to participate in a comprehensive clinical interview.

Directions to Huntsville Clinic
From I-565 East take Governors Drive exit and continue on Governors Drive. Go under Memorial Parkway, cross the railroad tracks, go through the yellow blinking light and NACS is on the left just beyond Princeton Road.

Going South on 231 take Governors Drive exit and turn left onto Governors Drive. Cross the railroad tracks, go through the yellow blinking light and NACS is on the left just beyond Princeton Road.
Going North on 231 take the Governors Drive exit and turn right at the traffic light onto Governors Drive. Cross the railroad tracks, go through the yellow blinking light and NACS is on the left just beyond Princeton Road.

From Highway 72 East, which becomes University Drive, turn right onto Church Street which is one traffic light after going under Memorial Parkway. Continue on Church Street, which becomes Gallitin, to Governors Drive. Turn right and NACS is on the right just beyond Harvard Drive.

From Highway 72 West, which becomes I-565 in Huntsville, get onto Memorial Parkway South and then take the Governors Drive exit. Turn left at the traffic light onto Governors Drive. Cross the railroad tracks, go through the yellow blinking light and NACS is on the left just beyond Princeton Road.
