Making Appointments and Contacting Us

Rett Syndrome patients are evaluated at the UAB Sparks Clinics located in the UAB Medical Center on the Southside of Birmingham. You can obtain Sparks Clinics Rett Research Program and Clinic information by calling one of the Sparks Clinics main numbers below:

SETTING UP AN APPOINTMENT
Most children who are seen in the Sparks Clinics have been referred to us by a medical doctor or health care professional, due to concerns with neurological processing and functioning. The child’s physician typically contacts our office with pertinent information to set up an appointment for neuropsychological evaluation. Our clinic does accept Medicaid patients. Medicaid patients may instruct their primary medical physician (PMP) to fax the Medicaid Patient First Referral to Sparks Clinics, care of Joyce Brown. The Medicaid referral should include the names of the patient, primary care physician, and parent/guardian, as well as the home address and phone number of the patient. The purpose of the referral should also be included on the fax. PROVIDING IMPORTANT INFORMATION
Once a child or adolescent has been placed on our schedule, there are certain steps we must take prior to the evaluation date. For the evaluation to be as thorough and helpful as possible, the clinic will need additional information from the child’s family, as well as other facilities where the child has been seen. The parent will receive forms for the family and the patient’s teachers to fill out, prior to the evaluation. Although these forms do take significant time and effort, we ask parents and teachers to be as thoughtful and thorough as possible when filling out our forms. They are a very important part of our evaluation. It is important that we also receive medical records and any prior psychological, educational, developmental, or intellectual testing which may have been performed in the past. Caregivers are encouraged to provide us with any additional information or records that may be important for a comprehensive evaluation.
