The tasks necessary to move from adolescence to adulthood, dependence to independence, and ultimately to interdependence, are difficult for everyone.  These tasks may be especially challenging for some individuals who have a disability that limits them in various ways.  However, achieving these life-goals is possible.

We, in the Lifelong Coordination Clinic (LCC), are committed to providing support and assistance to young adults and adults with disabilities, so they are able to realize their individual dreams and ambitions.

Services may be as minimal as providing an occasional checkpoint, offering advice and encouragement, or, as complex as providing daily medication supervision, financial overview, employment support, transportation, and other assistance tailored to a person’s specific needs.  The solid foundation of our efforts stems from the belief that everyone wants to live full and productive lives, and everyone has talents and abilities.  Our job is to support our clients as they take charge of their own lives, assisting them in reaching their goals and dreams.

One of the guiding principles of the LCC is respect for individuality.  We recognize that while there are common needs that many individuals share, there are numerous unique and specific ways to meet those needs.  We are committed to taking the time and effort to find the best ways to work with each of our clients.  We view ourselves as partners in decision-making, and invite all of our client’s family members and close friends to participate with us in providing the support necessary for success and happiness.
