Meet Your Neighbor

We Would Like to Hear From You!

It is very important to our project and to people with developmental disabilities that we hear from you. We would like to know what you think about the Meet Your Neighbor video. Please complete form provided below in the Word document format. The information you supply will be kept confidential and will be used to document consumer impressions and responses to the video.

Consumer Feedback Forms

Providing feedback is very simple and can be accomplished with a single e-mail by following the 3-steps below. On a regular 28-56k modem it will take about 15-20 seconds for the document to open.
1. Simply click on the document link to open in Word. Click here for Word document.
2. Enter the information in the form and then save to your hard drive.
3. Click here to send the e-mail. Be sure to attach the file you just saved.

Additional Options for Feedback

If you do not have a Word compatible program consider printing the Adobe Acrobat version below and mailing a hard copy response to the address shown below. You may also call Vicki Coffey at 205-934-2965 and have a form Faxed to you or you may provide the information over the phone.
